Centennial Kindergarten Registration

The principal and school staff will be on hand to answer any questions. Please bring your child's proof of birth, social security number and immunization records. Proof of birth is a state or hospital issued birth certificate, or passport

Medications
Students who require ANY medication (including over-the-counter drugs such as Tylenol, ibuprofen, etc.) during school must have turned in a district authorization form for the medication that has been signed by BOTH parent and physician. Authorization forms are available in the school office.  The parent or legal guardian must bring the medication and completed authorization forms to the school. Medication must be supplied in the original prescription container and properly labeled with the child's name. All medication must be kept in the office and will be administered by trained school staff.

Absence?
If your child is going to be absent for any reason, please call the school and leave a message. Our message line (360)428-6138 is open 24 hours.
We will call your home and/or emergency contacts if we do not receive a phone call from you each day that your child is absent.
If your child will not be in school or arriving late, please call the school office to leave a message. (360)428-6138.
